To qualify for PING!

1) Member student must be enrolled in curricular band/orchestra class at D97/D90/D200 in the Fall of 2025. (If not, please register with your district, then complete this form, or if not returning to curricular band/orchestra, contact PING.)

2) Renting an instrument or otherwise participating in instrumental music is a financial hardship.

Please provide information for the student's Primary Custodial Parent/Guardian (who the student lives with most of the time). If you would like to list an additional parent/guardian, add them in the Adult Account Information section, after you have completed your own Adult Account section.
