To qualify for PING!

1) Member student must be enrolled in curricular band/orchestra class at D97/D90/D200. (If they are not, please register, then return to completing this form, unless instructed otherwise.)

2) Renting an instrument or otherwise participating in school instrumental music is a financial burden to our family.

Please provide information for the student's Primary Custodial Parent/Guardian (who the student lives with most of the time). If you would like to list an additional parent/guardian, add them in the Adult Account Information section, after you have completed your own Adult Account section.
